It provides people with the opportunity to work with a coach (health professional) to understand, manage and improve their health, in particular around the risk factors associated with diabetes and its complications. The COACH Program庐 is a free service available to Tasmanians over the age of 18, who have: type 2 diabetes pre-diabetes a high risk of diabetes, including women who have had gestational diabetes coronary heart disease (heart attack, stent or heart surgery) a high risk of heart disease Find out more about the COACH Program庐.
